F. MILITARY LEAVE (USERRA)
A military leave of absence will be granted to employees who are absent from work because of service in the U.S. Uniformed Services in accordance with the Uniformed Services Employment and Reemployment Rights Act (USERRA). Advance notice of military service is required, unless military necessity prevents such notice or it is otherwise impossible or unreasonable. The leave will be unpaid. However, employees may use any available paid time off for the absence. Contact the Human Resources Department for more information or questions about military leave.
G. MEDICAL LEAVE
Employees that have completed their introductory period may qualify for an AURIA medical leave at the discretion of the Company. This medical leave is available to those employees that have not been employed 12 months or have exhausted all of their FMLA leave time. To qualify, employees must provide certification from a doctor indicating the need for the medical leave and the duration. Please see your Human Resources Department for details and eligibility requirements.
H. HEALTH AND WELFARE
The following is a summary of the benefit plans offered by the Company to full-time, eligible employees. All benefits, coverage and eligibility are subject to the terms of the respective plan documents and work location. Questions or concerns regarding the Company’s insurance and other company benefits will be determined by reference to the actual benefit plan document. AURIA offers a comprehensive benefits package that allows eligible employees the opportunity to make benefit selections best suited to their individual needs. Eligibility for the following plans starts 30 days after date of hire. While detailed information concerning benefit selections is available through Human Resources, benefit choices are generally available for eligible employees in the following areas: Medical/Dental/Vision You may elect coverage for yourself and your dependents. You are responsible for a portion of the monthly premium depending upon the plan you choose whether or not you enroll dependents and on your work location. Income Protection (Short Term Disability) Income protection is designed to provide employees with supplemental income for unpaid time away from work due to their own illness or injury.
