Family Security Plans (Life Insurance) Basic term life and basic AD&D insurance coverage is designed to provide employees with a minimal amount of life insurance in the event of your death. Supplemental Plans (Life Insurance) Additional life insurance and AD&D is available at a cost, depending upon insurability, to the employee and the employee’s spouse and dependent children under the Optional Term Life and AD&D and Dependent Term and AD&D Life insurance plans. Flexible Spending Account and Dependent Care Account Both a flexible spending account (FSA) and a dependent care account (DCA) are available for employees to enroll in. These accounts provide tax savings to plan participants by allowing them to pay certain premiums on a pre-tax basis, along with setting aside pre-tax dollars to pay medical and/or dependent care expenses not covered by insurance. Payroll dollars contributed to this plan are not subject to federal, Social Security or most state taxes. Tax Saving (Section 125) Depending on the benefits you select, premiums are shared between you and the Company. Payments are made through convenient payroll deductions with pre-tax or after-tax dollars. Benefits on a Reduced Work Schedule For purposes of benefit eligibility, “full time” employees are those that are scheduled to work 30 or more hours in a week. If business demands are such that your work week is reduced to less than 30 hours for 30 continuous days, your benefits and eligibility will be affected. Please note that your disability coverage will stop as of your last day worked as a regular full-time employee. The Human Resources Department will provide you details if this situation arises for you. Change in Status You will have the opportunity to change your benefits through the annual enrollment process. However, should you experience a change in employment such as termination or reduction of hours or family status change, such as marriage, birth or adoption of a child, divorce, death of a spouse, child no longer a dependent or certain changes in your spouse’s employment, you may cancel your benefits or change your level of coverage. Qualified changes must be submitted in writing to the AURIA Benefit Service Center as soon as possible, but no more than 60 days after the qualifying event. f you have any questions, please see your Human Resources Department.
